Congregational Care Ministry Committee meets 2nd Wednesday of the month in the Library at 4:30 p.m.

- Team Leader (or designee) receives the Prayer List from Staff
each week.
- The prayer list & updates from the team and staff are used to prepare the Congregational Care List (CCL).
- CCL & Prayer List e-mailed by 12 noon Friday to office, Deacons, Congregational Care Team & Staff.
- Beginning Sunday, the team member on call attempts contact with each family on the CCL to determine meals needed. Volunteers are contacted to provide the needed meal. (It is the option for on call member to provide the meals or delegate).
- Cards are sent where appropriate
- E-mail updates are done providing contacts made, meals provided, those no longer needing meals and known needs for the next week.
- Final attempt is made to contact remaining members on list prior to ending call each Saturday.
- Team leader contacts new members or small groups to maintain a list of meal volunteers.

The Christian Kitchen is always in need of volunteers to work in teams to provide meals, make phone calls and send cards.

- Volunteer visitors organized by Phil Orazi & contacted by church office when a member is hospitalized.
- The Deacons on Call are also contacted by church office and may decide to visit hospitalized members.
If you, or another member, are in the hospital, or going to be in the hospital, please contact a Pastor or the Church Office (770-979-9676) with the following information:
- Name of person
- Name of hospital
- When admitted
- Why (unless confidential)

- The office contacts the team leader(s) of a death in the Church family.
- The team member on call is notified with the intent of food being delivered ASAP.
- The family is contacted and arrangements are made for meal to be delivered.
- Offer is made by staff or team member for luncheon on day of the funeral.
- Luncheon can be at the Church or at the home of a family member.
- Whole CC team is notified of luncheon plans. Team leader organizes and divides responsibilities.
- If held at Church, the office is notified - Calendar is verified for availability & heat/air needs. Tables & tablecloths need to be located & arrangements made with team to set up & break down.
- Team members notified of service time, set-up location, and drop off point for food. All food should be freshly made.
- Set-up at Church, if appropriate, make coffee. Clean up.
- Team will set up parlor for family before funeral with water and coffee as necessary.
In the event of a death, please contact the church (770-979-9676), or a Pastor, as soon as possible.
The Pastors, Session, Deacons and staff members stand ready to assist you at any time.

- Serves as clearning house for volunteers who wish to help with repairs or building projects in the church. (Advertises through bulletin/newsletter/minute for mission)
- Does set-up/break down for Wednesday night suppers (recruits volunteers to help)
- Provides help with special needs are recommended by pastoral staff.
The committee welcome all volunteers (both men and women).
NO EXPERIENCE NECESSARY!

- Chair contacted by office as to new birth.
- Church office arranges for rose(s) to be placed on pulpit on Sunday following birth.
- Team member calls family to deliver rose. Baptism info brochures delivered with rose.
- Team member offers to provide a meal for family / determine any special needs.

Stephen Ministry
- Team Members provide one-to-one care to meet the needs of peoople experiencing a wide range of life circumstances or crises. (See further description of this fine ministry by clicking on the following: Stephen Ministry
Shut-In
Visitation
- Team members will visit their assigned shut-in member on a regular basis.
- Team members will deliver poinsettias at Christmas and Easter Lillies at Easter to their assigned member.
- Team Leader will follow-up with deacons regarding their shut-in visitations.
Members
Missing in Action (MIA)
- The contact team calls members who have not attended worship for several weeks.
- A new MIA list is generated and calls are made every four to six weeks.
Health
Ministry
- Meets monthly--2nd Tuesdays at 6:30 pm in Parlor (takes break in December)
- Chair plans agenda
- Officers: Chair, Secretary
- Follows holistic, wellness model (spirit, mind, body)
- Uses Gwinnett Medical Center and community to resource speakers and materials
- Plans and implements health related events
- Blood Drives (twice a year)
- Educational Events (Sex Ed, Parenting, End of LIfe Directives, etc.)
- Support Groups (Divorce, Grief/Loss, Cancer Support, Caregiver, Job Loss)
- Training Programs (CPR, First Aid, Babysitter Training)
- Plans programs that cover all age groups (infant-senior)
- Hospital Visitation monthly sign ups
- Congregational Homebound and Nursing Home residents monthly contact.
- Maintains card ministry (for folks on prayer list, hospital, sick or death in family)
